How to Treat Chronic Pain Without Opioids
A lot of people come to a pain specialist worried about one thing: that they will just be handed a prescription for opioids. Others are already on them and want a way off. Either way, there is good news. For most chronic pain, opioids are not the best tool, and there are treatments that go after the actual source of the pain instead of just covering it up.
Why Opioids Are Not the Answer for Most Chronic Pain
Opioids can dull pain for a while, but they do not fix what is causing it. Over time the body adjusts, so it takes more medication to get the same effect, and that path comes with real risks. For most spine, joint, and nerve pain, opioids also do not work that well compared to treatments aimed at the source. None of this is a judgment on anyone who has been prescribed them. It just means there is usually a better long-term plan.
Start by Finding the Source
The most important step is figuring out exactly where your pain is coming from. Back pain, for example, can come from a disc, a joint, a nerve, or the bone itself, and each one is treated differently. Once the source is clear, treatment can be aimed right at it. That is the whole idea behind interventional pain care.
Non-Opioid Treatments That Target the Pain
There is a wide set of tools, and the right plan usually combines a few of them:
Targeted injections. Epidural injections and joint injections deliver anti-inflammatory medication right where the pain is coming from.
Nerve procedures. Medial branch blocks and radiofrequency ablation calm the small nerves carrying pain from arthritic joints. For lasting nerve pain, spinal cord stimulation and peripheral nerve stimulation can turn the pain down.
Procedures for specific problems. MILD for spinal stenosis and regenerative options like PRP for certain joint and tendon problems treat the underlying issue.
Non-opioid medications. Anti-inflammatories and nerve-calming medications can help without the downsides of opioids.
Physical therapy and activity. Building strength and movement is part of nearly every successful plan.
