What to Bring to Your First Visit

  • Photo ID and insurance card

  • Referral from your primary care provider (if your insurance requires one - call us if you are unsure)

  • MRI, CT, or X-ray discs from outside imaging facilities (the team already has access to imaging from Health Images, UCH, SimonMed, and several other systems)

  • A list of your current medications, including dosages

  • Records from previous pain management providers, especially procedure notes from past injections

  • Any relevant surgical reports if you have had spine or joint surgery

What to Expect at Your First Visit

At your first appointment, Dr. Carrera's team will review your medical history, examine you, and go through your imaging with you on-screen. Wear comfortable clothing that allows easy access to the area of your pain. The team will explain what is causing your pain and discuss your treatment options. If a procedure is appropriate, the team can often schedule it before you leave.

For Referring Providers

Fax referrals to 720-455-3776. The team handles prior authorizations and insurance verification so your office does not have to. For patients with acute compression fractures or new disc herniations, call 720-455-3775 directly and the team will arrange urgent access.
